[IUG] Need pverify_web.html help

I have customized pverify_web.html (https://uncclc.coast.uncwil.edu:444/patroninfo) to accommodate LDAP authentication in the left column and native Innovative authentication in the right column.  Both authentication types work, but I'm having trouble 1) coding the page to retain last name & UNIV ID input when the page is rendered for new PIN input or other input error and 2) having the cursor placed in the PIN input box when the page is rendered for new PIN input (the cursor always pops over to the Name input box in the left column).

If any of you coding experts would show me the error of my ways, I would be very grateful.
