I understand and apologize for the confusion my initial message caused. Our
system is currently on 2009A, with the menus (file set) for that revision.
3 links on my LibCat home page ("My Account", Recommend a Purchase" &
"Inter-Library Loan") appear to use the same token or html file:
"My Account" - /patroninfo
"Recommend a Purchase" - /patroninfo~S0?/0/redirect=/acquire
"Inter-Library Loan" - /patroninfo~S0?/0/redirect=/illb
All three bring up the "Login" screen where it is possible to log in using
campus identification or library identification.

Not to be picky, but all three bring up *a* login screen, not *the* login screen (because there are more than one in all systems). In this case it's the screen associated with logging in to one's patron record (pverify_web.html), which is the file you need to edit. (FWIW, if you view the source code of a page and do a find in page for "this is", you should see an HTML comment that tells you what file is being used to generate the page.)

It appears that "patform" is being called, which I haven't been able to
find, and am thinking it's one of the tokens that cannot be edited.

See the manual page for pverify_web.html (107019). The patform token writes the entire patron verification form (seems odd that it would include the campus creds piece if you don't have the external patron verification product), but you can ditch that and use other tokens (see the "For advanced customization of the form" section). In which case, don't include the extpat tokens.
