Hi Naveed,
We use QR codes here at Providence College, but have not implemented them within our own catalog records. (Link to our QR site is contained in signature below.)
I do know of one or two institutions that have done this before though. The University of Bath comes immediately to mind. In fact, they were one of the first institutions to do so. I'll provide you with some brief contact information off-listserv, if you'd like.
