[IUG] Question about expired holds

We currently have our holds set to expire after 3 days on the hold shelf as we were getting a lot of students not picking them up when we had them on for 5 days. However we are still getting quite a number of expired holds which are never picked up even though we have sent the pickup email.
What I would like to know is how do other people deal with this problem (if you have it) and whether it is possible to use Millennium to invoke some sort of a penalty for expired holds?
Any information or advice gratefully received.
