Innovative help desk came through with an answer (and...yes it was in the
manual).

The answer is really unfortunate though. You must have two templates, one
for print and one for email. The print image used the local directory,
pulled in with the import, "image.gif" for example. The template for email
requires that a copy of the image live on a public site and you refer to it
as "mysite.org/directory/image.gif". It increases the workflow, in that
staff need to run two print prepares.

Enhancement request form, here I come!

Hello!

I am working on a template for a hold pickup notice. Nothing flashy,
really. I have the library's logo in the template. The image appears in
the preview and is successfully included in the printed document. In the
email the image does not show up and displays as a box with a red x in the
center, much the way a web page would when the image can't be found. We
are not seeing any sort of restrictions that would cause this in our mail
server or in the email client. Any thoughts you might have about why would
be appreciated!
