[IUG] Proxy Error from "Forgot your PIN?" link

I've got an open ticket with the Help Desk on this, but the solution has thus far eluded us.  When we click on the "Forgot your PIN?" link on the patron sign in page, (https://marinet.lib.ca.us/patroninfo ) it results in a Proxy Error.  Here's the text of the error message:
"Proxy Error
The proxy server received an invalid response from an upstream server.
The proxy server could not handle the request GET /pinreset~S3<https://marinet.lib.ca.us/pinreset%7ES3>.

Reason: Error reading from remote server"
I sure that this started when we upgraded our server hardware and simultaneously upgraded from 2009b 1.0 to 1.1.  Innovative did check wwwoptions, and this function now works again on the staging server, which has the 2009b example set, at http://marinet.lib.ca.us:2082/patroninfo.  (It was giving an "invalid function" error, on a much more attractive web page, but also didn't work before they looked at it yesterday.)

The wwwoptions for both PIN_Reset and PIN_Reset_Email are identical on both staging and live.  I've pored over the html and the pinresetrequest token, and can find nothing wrong.  Can anyone offer any insight on this, or has anyone seen this issue before?
