I would try 'editing' the record to get it to re-index. It sounds like
'volume' ended up scoped but not 'millionth'.

Go to our catalog, http://kentlink.kent.edu and perform a basic keyword search with the single term "millionth" using the default scope (Entire Collection). The 22nd record, for Leaves of Grass, is the object of the treasure hunt. (everything works)
Now change the scope to Special Collections and repeat the search. Only 4 records, but NOT including the desired one even though the bib and item locations should put it in the Special Collections scope. (something is not working!)
Now add the term "volume" to the search ("millionth volume", not quoted) and leave the scope at Special Collections. Even though theoretically you are narrowing the search, you now get 5 records, the extra one (the 4th) being the one we wanted in the first place. (scoping is working, but keyword??)
Any ideas?
