Jim,
We have been running Red Hat on IBM as a turnkey system since September
2008 and have had only a handful of failed backups in that time, but
they have all been within the last six months or so. It hasn't been
consistent enough for me to consider anything other than a tape anomaly.
Now I'll pay closer attention should it happens again.

IUG/Linux sites,
Since March 22rd our site has been plagued by failed backups/fail
initialize :
On the week of 3/22 UC had a failed backup - repeat attempts further
failed -we replaced the DAT drive and secured all new DDS tapes.
On the week of 4/3 UC had another failed backup - repeat attempts
further failed -we replaced the DAT drive.
On the week of 4/15 UCIN had another failed backup - repeat attempts
further failed till the same drive was reseated and a full backup went
off 4/17.
On 4/18 Given the repeat failures with no OS/III/hardware changes save
new DAT drives we decided to change ALL the hardware, but still
encountered failures on backup.
On 4/19 We obtained another new DAT drive (on our fourth in a month
period) dailies/fulls worked.
On 4/22. Inserting the next full backup for initialize and within the
III menus the backup initialize has frozen on "f" Tape Initialization
III agrees to actually look at the occurrence as all new DAT tapes,
fourth DAT drive, all new hardware and no OS changes. They find some
Oct. 2009 debugging flag which they believe was causing the problem.
Such is removed and backups commence normal for two weeks.
On 5/6 backup menu freezes with initialize. Back to III - this time
"update your Kernel". We update to the latest Kernel -reboot still
cannot initialize. We install our FIFTH DAT drive and have been fine
for a daily and a full since 5/7.
We presume such will not last.
Have any other III Linux sites been hit by such an series of backup
events? We here on are RH ES4 latest kernel.
