Our setup is very similar. However, if an item is missing, we do allow holds. Often times we find the item in question in the wrong place on the shelf or on a table where someone was using it. We have had occasion to never find a missing book and had to cancel the hold to discard the item.
New books have no item record attached and patrons can place holds on these.

We don't do paging for local patrons.
If it is available and on the shelf, go get it and check it out. No hold
necessary.
If missing, billed, on search, etc. we don't know where it is and
presumably the patron doesn't either. But if you find it, we will check
it out to you.
