[IUG] Paging requests crashing our circ system

We have a weird problem happening in our circ system. We have been
working with the help desk for months and are still puzzled. We
recently implemented title and item level holds in our system and
periodically these holds will crash our circ system. I am not a circ
person and I cannot figure out what is happening. Here are the details:

We are a closed stacks library that circulates materials to staff in the
building only. Patrons place requests in the web Opac for materials
listed as available. These requests show up in our paging lists and are
retrieved by library staff. When the staff tries to check out the
materials the system freezes and sends a message that says the server is
not responding and shuts down the module.

We have checked every possible thing we think it could be from patron
records to loan rules and have not been able to figure out what triggers
the problem. The weird thing is that it doesn't happen consistently.
Patrons with the same identical patron record settings asking for the
same I type books work fine but some patrons crash the system with their
request. I have discovered that the holds that crash the most are item
level holds.
