[IUG] Requesting off-site storage items

Does anyone, not allowing available material to be requested
(denyifavail), manage to use the Request functionality for requesting
off-site storage material?

We currently offer a non Millennium static web form for requesting
off-site storage items. This form is not prepopulated with data from the
bibliographic record, so it is clumsy for the patron to juggle two
windows and retype or cut and paste the relevant data from the bib
record to the request form.

So far we have not considered using the request functionality for this
purpose because the off-site material has an 'available' status, and we
only allow requests where items are 'not available'. 

Alternatively has anyone prepopulated a non-millennium request form with
data from the bib record?
