[IUG] Patron can place multiple requests on the same bib record

Hello,

We have a patron who found a way to place multiple requests on the same bib
record in our OPAC and naturally she is now placing up to 5 holds on the
hottest DVDs in our catalog:-(

When any of the staff tries to replicate the behavior we get "Request
denied." as we do not allow patrons to place multiple requests on the same
item/bib trough the OPAC.

We are using title - level paging.

Has anyone dealt with this issue before?
