At one time we had an open ticket for a similar problem.
The Help Desk told us the typical cause is a misconfigured item
template - one that has the "Number of Renewals" already populated
at the time of item creation. Since then, we have stumbled upon
another possible reason; some patrons who access their My Millenium
had created a very weird cookie which renewed all their items, simply
by logging into their account. Good luck tracking down your scenario.
