[IUG] WAM & non-functioning pverify2 form

Hello,
 
I am seeking advice why our pverify2 (WAM authentication) page is not
working. Upon clicking Submit, the form empties out. I do not seem to be
having this problem on my computer. I gain access to the requested database
without a problem. A person at one of our libraries can't get beyond the
form. Remote access outside this particular building is not available.
Probably some ip issue. Any clues?
