I do a keyword search on 'obesity or overweight' and see the titles
ranked by relevance. If I do another keyword search on '(obesity or
overweight) and (adolescents or teens)' the relevance ranking
disappears.

I think that multiple parenthetically-grouped strings in a query are considered a "power search", which breaks out of the RightResult display of search results. (Same as with using quotes or searching a "slice" of the keyword index.)
