[IUG] Modifying Bookings Login Form

We're implementing the bookings module, and I'm trying to customize  
the login form (pverify_bookings.html). Instead of using  
<!--{patform}--> I want to change some of the options.

Problem is, when I remove the token and use the individual login form  
tokens (e.g. <!--{extpatid}-->) the form won't work. It looks like  
this is due to the original bookings login form using something called  
"webbookform" which is not a part of the page itself.

Has anyone successfully modified by pverify_bookings page to use  
something other than the <!--{patform}--> token? If so, what, if any,  
special tokens did you use to enable the page to work?
