Thanks for your response. The situation we have could be partially
explained by batch checkin or marking the item lost, which would get the
item out of the patron's list of checked out items but leave the
replacement fee. However, I can't explain the fact that the item got
checked out to someone but still has a status of billed. If an item has
a status of billed and we check it out, the status is cleared
automatically upon checkout (I tried this on another item).

non-billed item (I don't have a billed one) and found that the system
worked correctly when I checked out an item to one patron and then tried
to check it out to another.
