[IUG] Customizing online forms

Hello,

I am new to this list. This is my first time using this list.
I have questions related to opac online or dynamic forms.
Can you proved me with examples of how you customized some of these forms? I am more familier with perl forms and scripting and php. I am interested in how you set the form methods and actions. 
I like to see code level and the form.
