[IUG] Statement of Checked-Out Items

Need some help regarding circulation notices. Hopefully someone else has
fought this battle and figured this out.
We'd like to have the max number of lines for our circulation notice text
(15) (according to the documentation that is the max), so that I can give
the patrons some response choices when we email them their "statement of
checked out items". I've changed Circ Option #69 to 15 for the height.
However, it prints the circulation info in the middle of the message text
(see below).
I also tried setting the Item 67 info (even though that seems to be for all
the other notices where it prints 8 items in two columns) -- changing lines
to start with line 39 instead of the old 31 but it still does the
following. [SIDE NOTE: To be perfectly honest, what I'd really like to
have is the check-out items notice print in the 8 x 2 column format like
overdues, but according to Innovative that is a "system enhancement"
request.].
Oh yeah, and if I look at the character cell display of the format it looks
fine (shows all the mmmmmm and iiiiiiiiiiii's in the right places for your
standard overdue 8x2 format), no errors.

Am I missing some setting somewhere, something staring me in the face and I
can't see the forest? Any clues? I didn't find anywhere in the
documentation where the checked-out notice is set up differently from the
others.
