If you are sorting your list using the generic call number or title (c
or t) tag instead of sorting using the MARC tag, you will get duplicates
if there is more than one call number or title field in the record.
I've had this happen several times and now only sort using the MARC tag.

Are you indexing call numbers from more than one place. At UNC, we
index from the holdings record and the item record. A call number
search pulls up duplicate hits but it is really the same record just
indexed 2 ways. Or if there are more than one item then we see the 3 or
more hits for each item.
