Ours is called "Form & Genre" (how creative!). We don't use "index"
because all our index choices are in a drop-down list (see
http://exlibris.colgate.edu/).
I'm not sure how much more explicit we can be in the name of the index.
We're working on wording for our "Catalog Help" page, but I don't think
many people actually use that. We're also trying to include information
for the Form & Genre index in Library Instruction sessions for people
who are most likely to use it (film studies, primarily). --ann

Have any of you who have a form & genre index come up with a better way
of describing it to the end user?
We'll be putting ours up next month, and I have no idea what to call it
so it makes sense -- especially since it isn't used in all cataloging
here.
