Statement of Charges notice questions

We are trying to print statement of charges from Millennium and we actually 
want to print them.

Problem #1:  the cents is getting cut off.
Example
Gone with the wind ..................................................  $45.

I tried changing the width of the form, but that did not do anything.

Problem #2: when the notices printed they did not print one per page, but 
printed continuously.

Any ideas?
