Check-in Strangeness: patron associated with not checked out ite m

OK, Circ Gurus out there, riddle me this.
 
Normal
I check in a (checked out) item in check-in mode.  I see the Barcode, Patron
name, Amount Due (if there is a fine), and the Status (number of days
overdue, etc).
 
Normal
I check in an item that has not been checked out, and I see the Barcode and
a status of NOT CHECKED OUT.
 
Strange
Under what conditions would I check in an item and see the Barcode, the
Patron Name, and the status NOT CHECKED OUT?  Items checked in before and
after come from this patron.  But look at the LPATRON field of the strange
item record, and the value in it does not match the patron name in the
check-in screen.
 
I have one branch seeing this problem periodically.  Either I'm missing
something extremely obvious, or something really weird is going on.  Can
anyone tell me which it is?
