How to diagnose Hold Problems?

Greetings,

We went live with Circulation in Feb on Silver.  We are finding that
there are questions about why a library was not notified of a hold or
other problems with the holds.  I would like to be able to investigate
these problems but because getting the item to the patron is our number
one goal,  the item is often checked out to the patron when I learn
there was a problem.   And that wips out any hold infomation that I can
find.   How do people handle these kinds of problems?  Is there
somewhere I can see old hold data without creating our own database of
the data?  (ie we would capture the files and store them in a SQL
database).
