paging problems -- share your pain

I'm looking to share some information with other sites having problems with
paging (specifically title-level paging) on Millennium Silver. We have been
working with the help desk to try to solve ongoing but somewhat intermittent
-pages not appearing on the paging lists;
-pages not rotating off the list after a day if they have not been filled;
-a single hold generates multiple pages;
-holds automatically cancelled, generating notices to patrons to consider
ILL, even though we have these items in the collection.
We have had these problems since we went live on Feb. 1. Our work with the
help desk and trainers has effectively eliminated all the user error
explanations. If you are experiencing any of these problems, please let me

A few notes on our system: we are on Silver, we allow patrons to place
multiple holds on the same title, but they can place only title-level holds
(unless it is for one volume of a multi-volume set, then they can choose the
item), staff can place multiple holds for one patron (the book club
feature), and we use title-level paging. We are a four-branch library
system. Patrons can choose any branch as their pickup location, and they
can cancel their own holds.
