Are you sure that all your "locations served" info is correct? My experience is that a paging slip will not be queued when the item location matches the location served of the login used to place the hold. In other words, if the system thinks you are working at a particular branch (based on login) and you place a hold on an available item that it thinks belongs to that same branch (based on the location code in the item record and its corresponding branch in the "location served" table), then "no paging slip for you" as the soup nazi would say.
The assumption is that you have the item in hand if you are a staff person placing a hold on it in the branch that owns it. Yeah, right. And I'll be sure to go get it and check it in to the holdshelf right after I answer this next call and check out 2 more people.
Less likely but still possible is that if you are using a login that doesn't match the branch you are in, weird things happen. Sometimes subs or staff who work at several different branches forget and login with another branch's login.
Or, if your locations served table has some omissions or errors, the system might erroneously think a particular item belongs to your branch and not page it.
Maybe I'm stating the obvious, but people keep talking about the pickup location and I don't think that has anything to do with it.
I think the bulk of our errors in this regard are staff not clicking "print paging slip" when prompted. Every single staff person has to get this right every single time for all the holds to queue to paging slips.

Sheryl,
Thanks for the info. We are finding that some hold on available items at other branches other than the pickup branch are also not being trapped on the paging list. And I can understanding the learning curve you mentioned for staff as it would be hard to understand why the holds functionality within the circ module doesn't work.
Thanks to all those who responded to my question.

Kim, we were new to Millennium a couple of years ago as well and I will tell
you that holds create a learning curve for staff. Here are a couple of
things to look at:
1. Holds placed in Millennium (staff mode) do not page if there is an
available copy at the pickup location.
2. We still have staff who assume the prompt to place the hold and print
the paging slip means that something will print at their local printer.
After nearly 2 years, I am STILL correcting that false assumption. I almost
wish that instead of "print," III could make it say "send" or anything else
but "print".
These were and continue to be our two biggest hurdles and we still do a
significant amount of holds cleanup (700-800 items) each month with our 80
locations. We're looking forward to the changes that will come with Release
2005 Priority Paging.
OPAC holds self placed by patrons have fewer issues. We do get a report
once in a while of holds not making it to the patron record, even had one of
those myself but have so far been unable to recreate the problem for the
help desk. I keep working on it, it seems to be happening most when there
is an on shelf copy at the pickup location but from what our users have
reported that isn't always true so I haven't been able to pin down any
definite pattern.
