For our institution it is typically an issue of communicating the down time to all appropriate parties, prior to the upgrade. We generally have not seen any issues following upgrades. Just monitor the behavior closely.
One thing I have gotten in the habit of doing is printing copies of all system options (for each modeule i.e. aqu, db, circ, OPAC, and serials) prior to the upgrade. After the upgrade is complete you can then go back and make sure nothing has changed. You can also use this to compare options that may have been added due to the upgrade.
