Serials are Vanashing

Help,

 

            Our Items records for serials are vanishing from the system.
The branches check them in, and create the item records.  Then when they
get the next issue the previous ones are gone.  If you scan the barcode
it will bring up the bib record but there are no attached item records.
The check-in cards show that they are received.  Has anyone seen this?
Hopefully it is a quick fix.
