Our public library has eliminated paper forms in one branch [except for
elementary school classes/group] by way of iii's web-based self-registration
module. The guest keys the data themselves which populates basic info
[name, street, city, state, zip, phone, birthdate, e-mail] right into
milcirc [but not into the formatted birthdate or zip fields] and assigns
them a 7-digit p number. Staff completes the process by checking for
duplicate registration, verifying their I.D., scanning a library card to add
the barcode to the record, adds pcode, ptype, corrects/edits their keying
errors, etc. We do not keep any paper copies. The signature panel on our
card, which they sign when issued, indicates they are responsible for the
use of the card, report lost card promptly. We have not had a legal
challenge or problems. In the old days when we attempted to retain paper
applications by keeping them in 3-hole binders, it was a nightmare of
paperwork, not worth the staff time and you never could find the one you
needed anyway. There aren't too many literary-inclined identity thieves out
there but we would ask the victim to show some indication the theft was
reported to police, credit bureau etc. and then write off the loss.

We print the responsibility statement on the card itself. The patron/parent
then signs the card in the presence of a staff member. At that point we
discard the application form. Patrons need look no further than the card
itself for verification that they have agreed to abide by our policies and
procedures.

How do libraries, without "hard copies" of patron registration files,
handle "legal liability?" One concern we have going completely
"paper-less" is how to hold a patron responsible for paying materials
checked out on his/her card, or one of his/her children's cards.

Peter,

We enter all information at the time of registration, no paper anything.
The customer signs the back of the card at the time of registration
agreeing to our policies and rules. We got rid of our paper forms years
ago and have never regretted it.
