Jeff,

Currently the "Modify Patron Information" page is not customizable, but 
according to the Millennium Release 2005 Limited Edition release notes on 
CSDirect, this page will be customizable with the 2005 LE release. Here is 
the working from the Release Notes:
   * The Modify Your Personal Information page will be 
library-customizable form.
