One thing to add: purchase alert is generated by the setting in the
circulation parameters. The default is if the item has been held for 3
times, the system will generate a purchase alert. But that number can be
set to any number the library deems fit. At our college, we route
journals and we hold journals for the next person on the routing list.
We set the number higher to avoid excessive purchase alerts.

Eric,
If I understand you correctly you would like to create a list of items
with 3 or more holds.
In Create Lists you select an empty list, click Copy, then scroll down
past the list of "real" lists and you will see several system generated
lists including one called "Purchase ALERTS". Copy this list to your
empty file and you will be asked whether you want to remove the file
you are copying from.
Note that an item is added to the list every time a hold is placed on
the
item as a third hold or higher. So the same item may appear multiple
times. These holds may or may not be active when you view the list.
Not sure if this is what you are looking for...
