create list and 008 and silver..

A question to those who have upgraded to Silver:

In create list, if you select bib as the type and choose marc21 008 as
the field to use, the list is very long and some field, such as DAT
TYPE, FormItem repeats itself, though the code before it, such as c5,
i7, etc, are different.  Does anybody know what those codes are?  For
instance, if I want to locate microform, do I use c5  FormItem  or j5
FormItem or I3 FormItem?
