I have mentioned our solution to this problem on the Listserv in the
past so I don't know if this is one of the answers that you were not
satisfied with when you searched the archives.
We use proxy rewrite and found that users of one of the cable ISP had
problems when using Internet Explorer and we found that when they went
into Tools - Internet Options - Connections - LAN settings , that the
Automatically detect settings box was ticked. Everything will work fine
after they remove the tick from the box. This is such a common problem
for us that we have instruction on how to do this on our database page.

Our patrons who use cable to access the Internet have been
reporting problems with authentication using our Innovative proxy
server for remote access.

I searched the listserv archives and shared the information I found
there with our staff member who fielded the calls and she was not
satisfied with the answer so I am raising it on the listserv again.
