Title-keyword &/or Subject-keyword search?

Hi,
 
I'm wondering if anyone out there in Innovative-land has developed title-keyword or subject-keyword search screens for their library WebOPAC?
I know you can do a title/subject keyword search through advanced keyword searching, but I'm looking for examples where the searches have been set up as a separate search options/pages.
