We are using 8 1/2 by 11 and stuffing in envelopes.  We are a consortia of 8 academic libraries so we have a limited population of users.  It woks fine for us as long as the libraries remember to print them.  We usually have less than 50 per library even if it has been a while for them to print.  Additionally, because the system only prints for those items still overdue, the number to print is often reduced then we have to remind the libraries to print fines and bills.

We recently modified to notice to include the patron barcode on the notice.  This helps in handling the notice when the patron comes into the library to discuss the items.
