We experienced this problem before. After analyzing the bounced email we noticed the problem lies with the inability to verify the origin of the email because our Millennium server does not show up as a legitimate server.
If your Millennium/Innopac server name is IIIservername.xxx.org and your ISP does not have this URL in their mail server, then the users' mail server will not be able to verify IIIservername.xxx.org. Hence the bounced email. I believe this is some kind of mechanism that some SPAM filter uses to stop relay mail service.
After we asked our ISP to put our server name on their mail server, we have seen a dramatic drop in our bounced email.

We asked Innovative to edit our email subject line so that it is more meaningful to our patron. At least, this will help our patron catch the email if it did end up in their junk mailbox.
