Thank you, everyone. I found that while I had no keyboard timeout with
this login, I did have Mil. Circulation set to remove initials on keyboard
timeout. Perhaps that was creating a conflict. I changed the setting so
that the initials do not time out, and we had one successful test in which
we saw the "offer-to-override" screen, so perhaps that was the problem.

but first check to make sure the person's initials/keyboard haven't timed
out. You should be getting a popup asking for initials and password if it
has, but none of the other modules were as polite about this and maybe
MilCirc joined them in the conspiracy of silence. ;-)
