We had a corrupted Millennium login that exhibited this behavior.  Even
though we deleted the login and then added it back (closing Millennium
in between) the login would not work.  Even though we had deleted the
login, some 'shadow' of it still existed and the corruption continued.

We finally solved the problem by creating an entirely new login with a
*different* name. 

You might try this.
