I don't know if you heard from John F. Kennedy University or not, but this is what we did there when I was still there:
1) Had Innovative create a separate Journal Title Index that would be populated only from the locally defined 599 field.
2) Ran a Passport for Windows OML script against a review file of predefined journal titles to copy the 245|a to the 599_0 field.
3) Went through records with alternate title entries and copied them over to 59910 fields where appropriate.
4) Used rapid update to populate the 690 field to assign locally assigned subject groups to journals with added |vJournals. subdivision.
The great thing about this method instead of basing it on existing 2xx or 7xx fields is that the library has complete control over the index and local subject headings to include only those elements that they want to include. This is particularly important in the subject groupings.

Was actually a pretty simple and fun project to work on once the logic was worked out.
