You can try looking at the self-booking function in your OPAC, but limit it
to faculty patron types. See sec. #105004, 101226ff. (esp. 101230) in the
manual.
It would be a trade-off between self-booking refusals to other patron types
and getting away from faculty printing out a form.
However, it is not clear to me if you can separate out the restrictions on
patron types between self-holds and self-bookings.
We do allow patron requests (holds) in our OPAC but have not tried the
self-booking function. Perhaps someone who has done this can tell you more.
