Linda --
You can purchase additional forms from Innovative, but they have (in the past) encouraged libraries to utlize local printers or copy shops to produce ones that they will be using. With a local print shop, you can customize the colors and specific policy information that is on the reverse. I would not change around too much as the system is really designed for these forms insofar as POs, claiming and cancelling.
Hope this helps.

Subject: Acquisition order forms
We are almost using up the purchase order forms Innovative brought us at
the training in April. Does anybody know where to order more of these
paper forms for acquisition? We are a Millennium Phase 2 user.
Thanks!
