An addendum to Bob's observation: from my own experience (with another proxy
solution, not WAM), you *must* proxy http://www.ncbi.nlm.nih.gov:80 (yes, it
does seem like <host> and <host:80> would be the same, but...) as well as
http://www.ncbi.nlm.nih.gov in order to succesfully proxy PubMed.
