Forms in use with My Millennium

Greetings,

I'm hoping that someone out there can help me with this procedure that I'm 
trying to do.
We are currently working on the My Millennium module specifically dealing 
with the Patron being able to Modify Personal Information.  We do NOT want 
our patrons to use the default Modify Personal Information process.  We want 
to set up our own form, which will link from the My Millennium page, to be 
filled out and sent to assigned staff to be verified and manually inputed.  
Has anyone else attempted this and do you have any other suggestions?
