We now use the proxy rewrite method but have just tried the proxy method
to access Cochrane Library from home and it worked fine.  Our forward
table entry is as follows.

1 > Token                  cochrane
2 > IP Address/Host Name   *.update-software.com
3 > Port                   80
4 > Service Level          0
5 > Verify                 2
6 > Valid Patron Types     xx,xx,xx
7 > Valid Patron Home Libs
8 > Database Identifier
9 > Database Name          Cochrane Library
