You're not crazy, after making changes to the WAM table, you must always restart both the Web servers and the Proxy servers. The WAM table even tells you this every time you make a change to it. This can be aggravating, but one thing I've learned is to always leave one of your Web servers running while you restart the others. Once they have all restarted, go back and restart the other one. If you restart all of them at the same time, anyone coming to your site at that moment will get error messages...
Keep in mind that restarting these servers will interrupt anyone that is using the proxy at the time and they will have to reauthenticate to continue using the proxy server, so it is best to leave it to the off-hours whenever possible.
