"OTF"

We have been using Mill-Circ for a little less than a year.  We have always 
had a few complaints from our cataloguer's regarding errors when checking 
out items "on-the-fly".  Lately, since we are using Millennium we seem to 
be getting even more complaints.  Does anyone else experience problems with 
OTF's and what do you do to try and minimize problems.
