Somebody correct me if I am wrong since it has been a long time but it seems when I did a barcoding project 10 years ago for a library that was going online for the first time, I contacted an organization that actually assigned these numbers for libraries around the country since some of these numbers represent the library. Usually all patrons barcodes begin with one number (e.g. 2) and all item barcodes begin with another number (e.g. 3) and the next four digits represent the institution.  I wouldn't have known this myself so I'm sure that either the library system vendor or listserv gave me the info. I'm sorry I don't have more information.  I don't remember the contact organization.
