I think you can customize all of the webpac by creating the page you want and then just adding the ACTION attribute to the form tag. Our comments page is one example: http://librus.hccs.cc.tx.us/screens/comments.html The ACTION attribute is the Innopac form URL. The form input can't change, of course. Our patron information request screen is not yet ready to go, but this is it so far: http://librus.hccs.cc.tx.us/screens/getmyrec.html The only problem with this one is that it reverts to the java form in case an error occurs, so I have to figure that one out.
